Hannah, a young girl with a fragile immune system, underwent a heart transplant just 11 days after her birth. She relies on daily medication that costs a staggering $1,000, which further compromises her immunity. While the family enjoys a vacation in San Diego, the team dedicates their efforts to creating a more supportive home and environment for the Grinnan family. To top it all off, Lee Ann Womack surprises them with a special performance at the end, bringing joy to the family.
